    I’ve looked at this case a bunch now. IMPA is at 107 – pretty significant improclination, and has mild crowding. Their ideal tx plan recommends non extraction and doesn’t even mention ipr.

    Any thought as to why? Is it because the patient is severe hypo and space closure tends to be harder? They don’t even mention IPR. Appreciate any thoughts!

    Dane, this is a popular case to discuss!

    I would be considering extracting L4s in this case to maximize advancement. That is not what they put as a solution but it’s what I personally would consider. Yes, space closure is tough on hypodivergent patients.

    One line of thinking I can see is this: mild crowding so just align man arch and and progress records to decide about extracting Lower premolars. Already a strong chin button and lower extractions will accentuate that a bit (though it can be treated during surgery).

    So you are thinking correctly as I see this case.
