When asked for ideal treatment plan… where does it end? For example for this case I listed everything from Phase I until maturity… is that overkill or better be safe than sorry?
My answer:
1. Phase I RPE treatment to correct transverse deficiency
2. Phase I protraction facemask treatment to correct sagittal discrepancy until positive anterior OJ achieved with slight overcorrection
3. Monitor growth and development after completion of Phase I treatment
4. Upper Hawley retainer after completion of Phase I treatment to maintain palatal expansion
5. Begin PhII tx when primary second molars are exfoliating
6. PhII treatment with fixed brackets
7. Torque control to reduce retroclination of lower incisors during PhII treatment
8. Avoid Class III elastics or Class III corrector during PhII treatment if possible in order to not overly retrocline lower incisors nor procline upper incisors nor increase the vertical dimension
9. Clear essix retainers at completion of PhII treatment
10. Continue to monitor growth and development until maturity to evaluate if patient is a candidate of combined orthodontic treatment with orthognathic surgery
11. Cessation of growth to be confirmed by superimposing serial cephs taken 6 months apart
