    Hi! I have a question regarding Case 2

    For prompt 1: how can you determine that there is a maxillary transverse discrepancy? This doesn’t look so obvious to me. There are no posterior crossbites and the maxillary arch doesn’t look so constricted?

    Also, for the dental problems. the answer states excess COW. How can you determine that without actually measuring the u6-6 width vs the L6-6.

    And for Prompt 9: what could you have done differently?

    Could answer be either discuss surgery or even tads?

    To your question on Prompt 1:

    100% agree that this is not an obvious transverse deficiency. I would be looking for the same things you mentioned – posterior crossbites, max constriction, or obviously deficient buccal corridors.

    What is highlighted in this case is a way to look at photos to see the transverse deficiency and the curve of wilson. Notice how when looking at the mandibular occlusal shots that we see more of the buccal surfaces and in the maxillary occlussal shots we see just the lingual surfaces to the molars. They aren’t the extreme flare we see with a deep curve of Wilson but the illustration is to key our eyes in to looking for this when we don’t have physical casts handy.

    To your question on Prompt 9: No, this ABO style prompt is not asking for an alternative treatment plan but what movements would be needed to improve the CRE/CMF score. There is a good example in the the new resource on the CRE method on the ABO’s website.

    Following up on with the last question, can you mention the 7s? The lower 7s are not aligned with the L6s. Also the upper 7s have alignment issues and MR discrepancies with the 6s.

      For sure! Always mention the 7s. Back when folks took their own graded casts, they found that people lost the most points with the relationships/discrepancies of the 7s. I’ve seen some people who rarely/never bond 7s. I’m on the other side – it’s rare I don’t bond 7s. 2nd molars are teeth too! Give them a great position and stability.
