  • Question on ABO Sample Case #2

On ABO sample case #2 (screenshots attached), it asks for the potential complications associated with the possible ankylosis
of the mandibular second deciduous molars. One of the answers listed was decreased arch length. Could you explain how the ankylosis could lead to decreased arch length? Thank you very much!

    Good question, V.

The loss of archlength related to primary molar ankylosis has to do with the permanent first molars. If the primary molars and ankylosed and in infraocclusion then the permanent first molars do not have the contact point of the primary molars for maintaining arch length. Instead, the permanent first molar tips forward into / over the ankylosed primary molar. So you need to hold the space with a LLHA, space maintainers or if not held you’ll have to upright the mandibular molars to correct the collapsing over the primary molars.

That makes sense! Thank you so much for the helpful explanation! 🙂
